Life adjustment and caring are some of the many problems facing persons with physical disabilities, persons with chronic illnesses and older people. Our Community and Day Care Service aims to facilitate community integration and to enhance their quality of life.

Services Introduction

Persons with disabilities, persons with chronic illnesses and the elderly are encountering numerous problems in their daily lives including care arrangement, repercussions and emotional disturbance brought by illnesses or disabilities, social role changes, job market re-engagement, and confidence re-establishment, etc. Our community and day care services strives to help them tackle these problems, thereby enhancing our service users’ quality of life, building healthy lifestyles, rebuilding self-confidence and establish mutual support network.

Core Services

  • Day Rehabilitation Centre (Hong Kong)

    Integrating “Western and Chinese Rehabilitation Medicine elements” and “Holistic Care” concepts, Lee Quo Wei (LQW) Day Rehabilitation and Care Centre has a cross-disciplines team consisted of physiotherapist, occupational therapist, social worker, nurse and Chinese medicine practitioner to conduct a holistic evaluation for our service users and to offer them with appropriate rehabilitation treatments and care services.

  • Day Rehabilitation Centre (Kowloon)

    Established in 2001, Cheng Tak Yim Day Rehabilitation and Care Centre (CTY) specializes in providing rehabilitative therapy and nursing care to cater the needs of clients with various chronic illnesses including stroke, dementia and brain injury, etc.

  • Community Rehabilitation Network (CRN)

    Community Rehabilitation Network (CRN) was launched to provide persons with chronic illnesses and their carers with holistic and diversified rehabilitation service. Adopted the “Chronic Disease Self-Management Model” from Stanford University in 2002, CRN aimed at continuously promote self-help and mutual-aid amongst our service users, as well as to cater for their mental and social needs with appropriate services.

  • Vocational Rehabilitation and Retraining Centre (VRRC)

    The Vocational Rehabilitation and Retraining Centre (VRRC) was found to provide vocational training and employment counselling services to help persons with disabilities casued by illnesses or injuries to resume re-engage the society.

  • Kids On the Block (KOB)

    The Society for the Relief of Disabled Children (SRDC) and the Hong Kong Society for Rehabilitation (HKSR) brought The Kids On the Block (KOB) from USA to Hong Kong. It is an educational puppet troupe to introduce the special needs and problems of children with disabilities to the wider world of school children, such that the participants can learn to concern about the society they are living in as well as to accept the differences of people.

  • Barrier-free Community Re-integration Project

    Persons with disabilities and persons with chronic illnesses tend to stay at home due to mobility problems, and they will gradually disengage from society and may even affect their relationship with family members. Barrier-free Community Re-integration Project aimed at helping our service users to re-engage to outdoor activities, that they can gradually rebuild their confidence to live the community life again.
