Parkinson’s disease is a chronic degenerative disease of brain nerve tissue. According to statistics, one in every 800 people suffers from this disease. Although there is currently no cure for Parkinson’s disease, it can be greatly alleviated through a healthy lifestyle, frequent exercise, and maintaining a positive mood, as well as medications, surgery, other auxiliary treatments, and rehabilitation support.
Service Introduction
The Jockey Club “Movin Care” for Parkinson’s Disease Project (2023-2026) received a donation from the Hong Kong Jockey Club Charities Trust and was co-organized by the Behavioral Health Research Center and School of Nursing of the University of Hong Kong and the Hong Kong Society for Rehabilitation. It aims to provide patients with Parkinson’s disease a comprehensive and sustainable community care service model for disease-related communities, promote “early attention to disease risks, early diagnosis and rehabilitation support, and early care of physical and mental health”, while promoting the participation of the entire society in caring for Parkinson’s disease.
In response to the needs of patients with Parkinson’s disease and their carers, the Hong Kong Society for Rehabilitation has applied the International Classification of Functioning, Disability and Health (ICF) in community macro analysis and service design, and implemented the Jockey Club “Movin Care” for Parkinson’s Disease Project. Through community rehabilitation support service, CRN connects services with people through case management model and personalized service matching, provides patients and their carers with comprehensive community care support, and improves their physical, mental, social and spiritual health.
