In order to make it easier for people in the community to access professional and reliable health information, the Hong Kong Society for Rehabilitation received support from the Hong Kong Jockey Club Charities Trust and launched the “e2care” project in 2016. “E2care“ aims to prevent and manage long-term diseases and creates an inclusive environment. It combines the online learning management system with the Community Rehabilitation Network membership management system to promote user participation in services and create an impact on disease prevention and long-term disease management.

1. Members Area
After creating an account, members can record health indexes in My Life Diary, including body fat, blood pressure, blood sugar levels, etc. They can also register and pay on the website to participate in various health management courses at various centers under the Community Rehabilitation Network.

2. 3-minute Health Risk Assessment

Regardless of whether you have pain, insomnia or high blood pressure, you can use this function for risk assessment. After completion, you can leave your personal information and a dedicated staff from the Community Rehabilitation Network will follow up on your situation in the future.

3. Online Self-Study Platform
You can choose courses according to your personal needs. For example, people with stroke or their carers can learn about various related areas according to different stages of stroke. In addition to the skills on how to care for people with stroke, they can also participate in spiritual activation courses on caring for emotional health, encouraging each other to accompany the sick family members on the road to recovery.

4. Information Sharing Station
There are a large number of programs with different themes, including various chronic diseases, healthy diet, exercise training, epidemic prevention information, etc. In addition to video programs, e2Care also has a chronic disease database and articles discussing different topics, using a simple and in-depth approach to supplement health knowledge for everyone.
